Dock sales in ohio

Not sure if this is the right forum for this question.
all of my brewing experience has been in states where we had specific forms to fill out to sell a keg to a private citizen or group that didn't have a license to sell.
Question is, does Ohio have any sort of paper work they require for a dock sale?

i've never seen any paperwork filled out for kegs sales from brewpubs/production facilities in the cleveland area. you may want to do something for keg and/or tap rental/deposit but its not required. keg treated just like a growler i suppose. brewing license allows for self distribution i believe.
