Any decent local sanitary fabricator can make that up. I have had some custom stuff welded up by specific mechanical for our system, so I would guess any other manufacturer could do it too. Might also want to page through ebay.

An alternative would be an instrument T like this http://www.gwkent.com/instrument-tee-1-1-2in-tc.html and a standard elbow. That should give the same profile when clamped together.

Well i buy from a S/S welder who sells fittings. So if it is some thing different. He makes it.
So take a tee and a elbow and weld it together.
