PET 20ltr keg exploded

Hello !

We are examining an explosion of a PET 20ltr Keg that exploded after it was put in the dispenser and before tightening of the upper dispenser cover. The keg was full of beer.

Any input as to the possible reasons of this explosion would be greatly appreciated.

Update

After our extensive search, it seems that during the PET bottle manufacturing process, the plastic bottle was not manufactured correctly as it did not have uniform wall thickness, thus, the point where the bottle wall was thinner ruptured and the bottle exploded.
